Find a Doctor
Use our online directory to find in-network doctors, hospitals, pharmacies, labs and specialty services. These providers can help you get care when you need it.
You can search for providers by entering:
- The provider's name in the search box.
- Your child’s member ID number to find providers covered by the plan. You can also view specific providers covered by our CHIP plan using the “plan name” box.
- Your zip code to search for providers near you. Select the desired search distance or by specific county.
You can also select the type of specialty from the listed categories.
As a Health Partners Plans CHIP member, your child can visit a doctor at any time and from anywhere for non-emergency medical conditions. You have virtual access to Jefferson providers 24/7 with JeffConnect. Download the JeffConnect app from the App Store or Google Play to get started.
You can also use Teladoc®. This is a phone and video service that connects your child with doctors virtually. These consultations are free for Health Partners Plans CHIP members. To register with Teladoc, visit Teladoc.com. Have your child’s member ID number ready.

You can take your child to an urgent care center or walk-in clinic for minor conditions. You don’t need an appointment.
Urgent care centers can do most lab work, take X-rays, place IVs and apply stitches. Walk-in clinics offer convenience but offer fewer services.
