JeffConnect is a quick and easy-to-use option for care for needs like colds, flu, cough, fever, urinary tract symptoms, infections, minor injuries and prescriptions. You can also use JeffConnect if you want a provider’s opinion about whether or not you should go to the ER.

Teladoc® is a phone and video service that connects you with doctors who can help you with many non-emergency medical conditions.
Teladoc consults are free for HPP members.

If necessary, a Teladoc doctor can provide a short-term prescription that you can pick up at your pharmacy.
If you are new to Teladoc, you’ll need to set up an account by calling 1-800-Teladoc, downloading the mobile app or by visiting Teladoc.com. Have your Health Partners member ID number ready. After you register, you will need to complete your medical history. These are the same types of questions you answer for any doctor to give them the information they need to provide a diagnosis.
Once you’ve registered, you can call or go online anytime to request a Teladoc consult. A doctor will call you back, usually in about 10 minutes.
If you have a life-threatening medical emergency or if you need medical help right away, call 911, or go to the nearest emergency room.
Teladoc does not take the place of your primary care physician but can be a convenient solution for non-urgent illnesses.
