To see the average monthly cost per child based on income level, view the CHIP income guidelines chart. If your household income is below the lowest amount listed in this CHIP income chart, your child may be eligible for Medicaid instead of CHIP.

After you apply for CHIP, you will receive a letter to let you know if your child is eligible. The letter will include information about your child’s eligibility for free CHIP, low-cost CHIP, or full-cost CHIP. Monthly premium rates are below.

Health Partners Plans CHIP accepts monthly premium payments such as checks, money orders, credit card or debit card payments. Your child’s premium is billed and due before the coverage month. 

Premiums are regular monthly payments you must make to keep your health care plan active. You must pay your monthly premium even if you don’t need medical services during a specific month. Learn more about monthly premiums for Health Partners Plans CHIP members in the chart above. 

Copays are flat fees you pay for covered services like visiting the doctor or filling a prescription.
