Diabetes Prevention Program

Have you been told you’re at risk of Type 2 diabetes? If you have, help is available, and Health Partners Plans gives you access to the Diabetes Prevention Program.

What is Prediabetes?

If you have prediabetes, your blood sugar levels are higher than normal, but not high enough for a type 2 diabetes diagnosis. Prediabetes is a serious health condition that increases your risk of developing type 2 diabetes, heart disease, and stroke. You can have prediabetes for years but have no symptoms.

Diabetes Prevention Program

The Diabetes Prevention Program (DPP) is a lifestyle change program to help participants with prediabetes make healthy lifestyle choices and achieve weight loss. Throughout the program, you will learn how to increase physical activity, eat healthily, and lose weight to help prevent or delay diabetes.
